Somewhere in the last few years "contains ceramides" stopped being an ingredient claim and became the entire argument for a moisturiser. Which is odd, because the research that made ceramides famous is mostly a paper about why one lipid on its own isn't enough.
What the barrier actually is
The usual picture is bricks and mortar: flattened dead skin cells as the bricks, a lipid matrix as the mortar between them. That mortar is not one substance. It's approximately half ceramides, with cholesterol and free fatty acids making up most of the rest, arranged in ordered layers. Water leaves through gaps in that arrangement, and irritants come in the same way.
So a damaged barrier isn't abstractly "weakened." It's short of specific lipids, in a specific arrangement, and that shortfall is why previously-fine products suddenly sting.
Why the Ratio Matters More Than the Ceramide Count
This is the part the marketing skips. In the 1990s a group at UC San Francisco -- Man, Feingold, Thornfeldt and Elias -- worked out what mixture of the three lipids actually speeds recovery in damaged skin. Their optimisation work found that an equimolar 1:1:1 mixture permitted normal repair, and that pushing any one of the three to roughly three parts against one of each of the others accelerated it. A follow-up study in aged skin found the same pattern held.
The uncomfortable half of the finding: incomplete mixtures didn't just work less well. Lipids applied without their partners delayed barrier recovery compared to doing nothing. A ceramide poured into a formula with no cholesterol and no fatty acids to sit alongside is, on the evidence, not a barrier treatment.
None of which means a ceramide cream is useless. Most decent ones aren't single-lipid formulas -- cholesterol and fatty acids are cheap, standard emollient chemistry and are usually in there under names like cholesterol, stearic acid or a plant butter. It does mean the number on the front of the box ("5 ceramides!") is close to meaningless on its own.
What this changes about what you buy
Read the ingredient list past the ceramide. If the only lipid in the formula is a ceramide and everything else is water, humectants and silicone, it will feel lovely and do less than the packaging implies.

Desert Free's Cactide Double Repair Serum ($54) is the more serious option, built around lipid replacement rather than around one hero ingredient.
If your barrier trouble started at the cleansing step -- tightness immediately after washing, which is the most common way people damage their own barrier -- the fix is upstream. SSUNSU's Water Barrier Cera Ginseng Cloud Cleanser ($38) is a low-strip cleanser that isn't undoing the lipids you're about to apply. And PURITO SEOUL's Wonder Releaf Centella Cream Unscented ($27) is the pick if inflammation is running alongside the dryness, since it pairs ceramide content with centella.
How long it takes
Barrier recovery is measured in days to weeks, not applications. Skin that stings on contact usually settles within a few days of consistent lipid replacement and no actives. Full recovery of the water-loss measurements takes longer. If you're two weeks in with no change, the thing damaging the barrier is probably still in your routine.

Stop counting ceramides and start checking whether the formula has anything for them to sit alongside -- a cholesterol, a fatty acid, a plant butter. One lipid on its own is a claim, not a repair.
